· Employed -- Base salary: $215,000 plus: RVU incentive model on basis of average WRVU of 3,872 Other Information · Current panel of 3,500 patients · 700 allergy injections · 3,400 square foot office, 6 exam rooms · 55% percentage of practice is Pediatrics · Required Board Certification or currently in process Other Benefits · Five days of paid CME per calendar year · $3,000 for hospital approved professional society dues or continuing education expenses · Relocation reimbursement assistance of $10,000 · Thirty one days of paid time off each year to include vacation, personal time, holiday time and short-term sick · Life insurance with a face value equal to annual base salary (after one year of employment) · Health insurance, individual or family 80/20 split on the cost · Dental insurance plan · 401-K program, available after 90 days of employment · 401-K employer match after one year of employment · Employer tax-deferred annuity program, after one year of employment; approximately 6% contribution · Reimbursement for NYS licensure, and DEA registration fees ·
