Our Centra PACE Program

 

Centra PACE (A Program of All-inclusive Care for the Elderly) is a program of the Commonwealth of Virginia.

Centra is proud to offer an exciting new service to the community. This model of healthcare has been in existence for over 30 years throughout the United States. The program focus is on prevention, wellness and the belief that it is better for seniors with chronic care needs and their families to be served in the community whenever possible. In Virginia, the first PACE Center was opened in 2007. Centra PACE is one of 15 programs in Virginia.

The Centra PACE mission is to enhance life for frail older adults by providing care and services from a team of compassionate, qualified professionals. Centra PACE strives to achieve this mission by empowering participants to live in their homes and in the community for as long as it is medically and socially safe. Centra PACE is a service of Centra, a nonprofit regional health care system which has been nationally recognized for its quality, service and safety.

Centra Medical Group is part of Centra Health, which is a regional nonprofit healthcare system based in Lynchburg.  With more than 6,000 employees, a medical staff of nearly 500, and 150 employed physicians providing care in 38 locations, Centra serves over 380,000 people throughout central and southside Virginia.
