Our geriatricians are often the primary physician for older adults and help to coordinate the care of the additional services and specialists needed.  Our services include:

Consultations

Our geriatricians will conduct a comprehensive assessment to identify the signs of dementia and other geriatric conditions. Additionally, they will provide:

• Memory assessments
• Fall and balance evaluations
• Medication management
• Assistance with complex Geriatric syndromes

Coordination of Care

Hartford HealthCare Medical Group’s Geriatrics Team collaborates with other Hartford HealthCare services in our network to assess and support families and caregivers in a number of ways:

• Dementia experts with The Hartford HealthCare Center for Healthy Aging help educate caregivers about patients’ behaviors.
• Close coordination with Hartford HealthCare Rehabilitation Network and Hartford HealthCare at Home can further refer needed patients for gait and balance training.
• The team-based, resource-rich care provides an excellent quality of life for both patients and their families.

R.E.A.C.H.

Resource for Elderly And Care at Home

The R.E.A.C.H. program works closely with your primary care provider to send an experienced advanced practice provider to your home for health visits. If you have difficulty getting to the doctor’s office, or are a high risk for hospitalization or readmission, R.E.A.C.H. will provide you with the highest quality of care you need without leaving your home.
