*Responsibilities*
* Conduct comprehensive evaluations of patients to assess their physical abilities and limitations.
* Develop individualized treatment plans based on patient assessments and goals.
* Implement therapeutic exercises and interventions, including trigger point therapy and orthopedic rehabilitation.
* Monitor patient progress and adjust treatment plans as necessary to ensure optimal outcomes.
* Educate patients and their families on exercises, techniques, and lifestyle modifications to support recovery.
* Collaborate with other healthcare professionals to coordinate patient care and ensure comprehensive treatment.
* Maintain accurate documentation of patient progress, treatment plans, and outcomes.
* Stay current with advancements in physical therapy practices and participate in ongoing professional development.
*Requirements*
* A degree in Physical Therapy from an accredited program.
* Valid state licensure or eligibility for licensure as a Physical Therapist.
* Strong knowledge of anatomy and physiology.
* Experience or knowledge in home community care is preferred.
* Familiarity with orthopedics, pediatrics, inpatient settings, and clinic operations is advantageous.
* Ability to create effective care plans tailored to individual patient needs.
* Proficiency in various therapeutic techniques, including trigger point therapy.
* Excellent communication skills for effective interaction with patients, families, and healthcare teams.
* Compassionate demeanor with a strong commitment to patient-centered care.
