POSITION DESCRIPTION - PHYSICAL THERAPIST
*REPORTS TO: *Chief Operating Officer
*CLASSIFICATION:* Non-exempt
*PRIMARY RESPONSIBILITY: *The Physical Therapist is responsible for providing specified services to home care clients according to MDH guidelines and MN Home Care Statutes.
*JOB RESPONSIBILITIES:*
* Provides an admission assessment for all new clients:
• Completes all admission documents
• Obtains physician orders, as appropriate
* Assures that the assessment and physician orders are appropriately updated
* Consults with clients to learn their physical condition and symptoms
* Creates individual treatment plans that can be carried out at home
* Outlines treatment goals and methods to be used
* Monitor client progress
* Utilizes techniques like functional training and exercise, medication, diet changes, and specialized equipment that is designed to treat and alleviate pain
* Coordinate with other healthcare professionals, including RN on staff
* Completes discharges and transfers for clients when needed
* Communicates with families, responsible parties, case managers and other health care providers regarding the clients when indicated
* Documents, as indicated, in the clinical record
* Is aware of and uses good safety practices, including but not limited to:
* Body mechanics
* Fall prevention
* Reporting of defective equipment or hazards
* Wiping spills or wet floors immediately
* Using Standard Precautions and appropriate infection control measures
* Knowledge of emergency procedures
* Reporting incidents/accidents to the Director of Nursing or Administrator on the day of occurrence
* Participates in care conferences with other members of the health care team
* Maintains absolute confidentiality of all information pertaining to clients, families, and employees
* Promotes a safe and comfortable environment for the client and family
* Demonstrates excellent written and verbal communication skills
* Recognizes and manages own stresses, which may affect work performance
* Participates in team meetings
* Participates in in-service educational programs, as required by state regulations
* Performs additional duties as assigned by the Chief Operating Officer
* Adheres to all agency policies

*EXPERIENCE/JOB SKILLS REQUIREMENTS*
• Graduate of accredited physical therapy program and licensed as a physical therapist in the State of Minnesota
• Experience in home care or assisted living preferred
• Supervisory experience preferred
• Must be able to read, write, comprehend and carry out directions and instructions
• Able to function independently
• Good health status and emotional stability
• Demonstrate a strong commitment to service excellence
WORKING CONDITION REQUIREMENTS
: (Occasionally means 1-2 hours of an 8 hour workday, frequently means 2.5 to 5.5 hours of an 8 hour workday and continually means 6 to 8 hours of an 8 hour workday)

Pay: From $55.00 per hour
