This role offers the unique opportunity to provide personalized, one-on-one care in the comfort of patients' homes while enjoying the autonomy and balance that home health provides.

Key Responsibilities
Evaluate speech, language, cognitive-communication, and swallowing disorders in home-based patients
Develop individualized treatment plans based on physician orders and clinical assessments
Provide direct speech therapy services to help patients regain or improve communication and swallowing abilities
Educate patients and family members on exercises and strategies for ongoing progress
Document patient progress, update care plans, and coordinate with interdisciplinary team members
Participate in care conferences, in-service training, and performance improvement activities
Maintain compliance with HIPAA and company policies for patient safety and confidentiality
Requirements:
What We're Looking For
Master's degree in Speech-Language Pathology
Current SLP license in the state of New Mexico
Certificate of Clinical Competence (CCC-SLP) from ASHA
Prior experience in home health or medical settings preferred
Strong interpersonal and organizational skills
Self-starter who thrives in an independent, mobile care environment
Valid driver's license and reliable transportation
