*SUBJECT: Job Description - Physical Therapist*
*POLICY:*
To provide Physical Therapy consultations and services to the patients through Visions Home Health, LLC, as requested by the patient's physician.
*PROCEDURE:*
*Reports to:*
* Home Health Director
* Patient Care Coordinator
* RN Case Manager
*Job Summary:*
* Essential Job Functions:
* A physician's order must be obtained for specific physical therapy services.
* To develop a plan of treatment in conjunction with the physician and based on the patient assessment.
* Advises and consults with the family and other agency personnel.
* Provides exercise, treatment, and recommends durable medical equipment when applicable to maximize functional independence in the home setting.
* Provides patient and family instruction on care and use of the durable medical equipment.
* Will provide the agency with a Plan of Treatment, progress notes, and billing information in a timely manner according to agency policy.
* Plan of care to be completed and submitted to Visions Home Health office by the end of the next business day following the initial visit.
* If no further visits are planned, a record of the evaluation including reasons for non-treatment are to be submitted to Visions Home Health office by the end of the next business day following the initial visit.
* Clinical progress notes to be submitted to Visions Home Health office by end of 2nd business day following patient visit.
* Billing invoice will be submitted to the agency within 10 days of the billing month. \* Referrals will be contacted within one working day of the referral receipt, and every effort made to schedule the home visit within two working days of the referral.
* \* Completes the Plan of Treatment within one business day of the initial visit if further visits are needed.
* \* There will be a record in the personnel file of the current contract and requirements.
* \* Maintains ethical practices and confidentiality, personal and professional integrity, and good physical and mental health.
* \* Will participate in or hold in-services for the home health agency as deemed necessary.
* \* Will attend and participate in Home Health multidisciplinary meetings concerning patient review of services, needs, and goals.
*Other Job Functions:*
* Assess and evaluate the patient's needs for physical therapy.
* Instruct the patient in performing activities of daily living.
* Assess and evaluate structural barriers.
* Therapeutic exercise programs.
* Cardiopulmonary physical therapy.
* Gait training.
* Ultrasound therapy and electro-therapy.
* TENS (transcutaneous electrical nerve stimulation).
* Prosthetic training.
* Mobilization/myofascial release.
* Iontophoresis.
* 32 hours every 2 years of continuing education.
*Requirements:*
* Graduate of an approved school of Physical Therapy with degree in Physical Therapy.
* Certified or licensed in the State of Idaho.
* Current Idaho driver's license and an automobile.
* Current CPR certification.
* Participates in in-service programs, in addition to mandatory CPR, safety and infection control programs as recorded on the educational record.
*Physical Requirements:*
* Requires lifting up to 50 lbs on regular basis.
* Requires sitting and driving frequently.
* Requires sitting, standing, bending, stooping frequently when assessing and working with patients.
* Requires wearing appropriate attire and close-toed shoes in the clinical setting.
