*Job Summary*
The Registered Dietician is responsible for planning, supervising, directly participating in and documenting all aspects of the nutritional care of AgeWell PACE participants. Performs nutritional assessment and provides nutrition education for AgeWell PACE participants requiring intervention. Acts as a resource to the health care team. Coordinates serving of meals, monitors/coordinates kitchen facilities to ensure standards are met. Coordinates with catering services to ensure catered meals meet needs of AgeWell participants. Participates in Quality Assurance activities. Participates in (sponsor) committees, where appropriate. Provides education to other health care professionals, as well as outside groups, as appropriate.
*Position Responsibilities*
* Performs initial nutrition screening utilizing various lab values and physical assessments and obtaining primary care provider's orders to develop care plans and monitor participant progress.
* Keeps confidentiality of client records, reports and discussions.
* Interacts with health care team members, as appropriate, to accomplish these tasks.
* Provides nutrition counseling for AgeWell PACE participants, using appropriate materials.
* Discusses diet principles and other information as appropriate about client's food preferences, methods of preparation and cultural and environment.
* Participates in the development and revision of the client's plan of care as a member of the interdisciplinary team.
* Educates the client's family or other caregivers about the need for and preparation of therapeutic diets as part of the treatment plan.
* Participates in educational activities, developing or presenting monthly group classes; developing or selecting/improving educational materials for participants and appropriate care teams.
* Completes required order information in conjunction with AgeWell and monitors receipt of items.
* Approves food/non-food purchasing (including bulk food orders, nutritional supplements, paper goods and external food/delivery customer bills) monthly and forwards information to Finance Department.
* Works with management team to prepare annual budget and manages appropriate areas of the budget.
* Identifies nutrition service delivery problems and uses good judgment in their solution.
* Participates in formulation and maintenance of AgeWell policies and procedures.
* Performs additional tasks as directed.
* Ensure proper use and protection of information assets by complying with the organization's information privacy and security policies to protect assets from unauthorized access and by reporting any security events or potential events or other security risks to the organization.

*Experience & Education*
* Masters of Science, Bachelors of Science or Dietetics Degree and registration with the American Dietetics Association
* Two or more years of experience as a clinical dietician either in a hospital setting or outpatient/rehabilitation facility.
* Experience with adult learning principles and outpatient counseling.
* Exercise sound judgement and exhibit strong decision-making and problem-solving skills.
* Excellent writing, proofreading, and editing skills.
* Proficient with software including Microsoft Word, Excel, and Outlook.
* Experience working in a nonprofit, strongly preferred.
* Experience working in a healthcare setting, preferred.
* Bilingual in English & Spanish, preferred.

* _Physical demands: _Employee must be able to sit for extended periods of time. Employee will also be actively present throughout the PACE center, engaged with employees, participants and with staff. Employee is required to use hands and fingers, especially for typing on the computer and using the mouse. Employee must be able to talk and hear, particularly for daily interactions with staff and elderly participants.
