*What You'll Do*
* Provide in-home respiratory assessments, treatment, and follow-up care for patients with COPD, asthma, CHF, ventilator or tracheostomy needs, and other chronic respiratory conditions.
* Educate patients and caregivers on equipment operation, oxygen safety, and preventive strategies.
* Partner closely with nurses, physicians, case managers, and family to coordinate seamless transitions from hospital to home.
* Monitor patient progress, report changes promptly, and support physician-directed Plan of Care/Treatment for optimal outcomes.
* Serve as a trusted advocate and motivator - empowering patients to take control of their health journey.
*What You'll Bring*
* Current IL Respiratory Therapist license (RRT or CRT) and BLS certification.
* Previous experience in home health, subacute, or pulmonary rehab is strongly preferred.
* The ability to supervise clinical personnel and have a basic understanding of insurance requirements (Medicare, Medicaid, and private insurance).
* Comfortable working independently while maintaining strong collaboration with the care team.
* A genuine passion for meeting patients where they are - literally and emotionally.
