*Social Worker, LMSW*
Community PACE at Home in Newaygo is seeking a Medical Social Worker to join our health care team. We need a person that will be responsible for the social work services to the participants of the Community PACE at Home program. This position is in-person, Monday through Friday both in the clinic and regular home visits are required. Our service area includes multiple counties. You will be required to travel within our territory on a regular basis. The counties included are Newaygo, Montcalm, Mecosta, Osceola, Lake, Mason, Oceana, and Manistee Counties
The Program of All-Inclusive Care for the Elderly (PACE®) model is centered on the belief that it is better for the well-being of seniors with chronic care needs and their families to be served in the community whenever possible. Community PACE is proud to deliver all the medical and supportive services needed. Our PACE program can provide the entire continuum of care and services to seniors with chronic care needs while maintaining their independence in their home for as long as possible.
By joining our team, you will be responsible for the direct social work case management services to the participants of Community PACE at Home. You would work under the direct and indirect supervision of the Executive Director. You would provide social services support such as participant screening, case management, counseling, and referral.
Requirements:
• Master's Degree from an accredited School of Social Work.
• 2 years' experience in a health-related area. Minimum of 1 year experience working with the frail elderly population. Experience working with Medicaid services and populations strongly preferred.
• LMSW, valid MI Driver License and vehicle, CPR and First Aid certification.
• Analytical thinking, and problem-solving abilities. Includes home visits to private homes. Only act within the scope of his or her authority to practice.
• Immunizations: You must be medically cleared for communicable diseases and have all immunizations up to date before engaging in direct participant contact.

Key Responsibilities:
• Provides individual and family counseling, develops, and leads group counseling and activities.
• Refers patients and families to appropriate community agencies or facilities, acts as liaison with such organizations and advocates for participants.
• Consults with and advises staff members as to the relationship of social, emotional, and cultural factors to health and medical care, and as to the availability of social services in the community.
• Participates in Program and Policy development of Social Work component.
• Facilitates Medicaid application and re-determination process with Michigan Department of Human Services.
• Documents patient changes appropriately in the medical record.
• Attend staff meetings and take part in patient care planning.
