*Position Summary*
We are seeking a skilled and compassionate *Physical Therapist (PT)* to join our growing home health team. The PT/PTA will provide high-quality rehabilitative care to patients in their homes, develop individualized treatment plans, and work collaboratively with physicians and the interdisciplinary care team to ensure positive outcomes.
*Responsibilities*
* Conduct initial evaluations and develop individualized Plans of Care.
* Provide physical therapy services as ordered by the physician and in accordance with agency policies.
* Teach patients and caregivers safe and effective ways to improve mobility, strength, and function.
* Reassess and update the Plan of Care as needed, including after hospitalizations or condition changes.
* Document all patient care accurately and in a timely manner per agency and state standards.
* Communicate effectively with physicians, RNs, LVNs, and office staff regarding patient status and progress.
* Participate in interdisciplinary case conferences and quality improvement initiatives.
* Supervise and provide guidance to PTAs as required.
* Promote safety, independence, and quality of life for patients.
* Maintain professional growth and participate in ongoing education and training.
*Qualifications*
*Required:*
* Current *California Physical Therapist*
* Valid *Driver's License* and reliable vehicle with insurance
* Ability to travel to patient homes across Los Angeles County
* Strong interpersonal and communication skills
* Ability to lift, transfer, and assist patients as needed
*Preferred:*
* Experience in *home health, hospital, or outpatient rehabilitation* settings
* Bilingual communication skills (any language)
* Familiarity with electronic medical records (EMR)
*Required Credentials (must remain current):*
* *California Physical Therapist* Proof of License
* Driver's License & Auto Insurance
* TB Test / Chest X-Ray
* Physical Exam
* CPR Certification
Pay: $90,000.00 - $130,000.00 per year
