*Job Description*

We are seeking a skilled and motivated Pharmacist to join our dynamic team. The Home Infusion Pharmacist plays a critical role in coordinating, compounding sterile medication, and verifying medications for home and alternate-site infusion therapy. This professional collaborates closely with physicians, nurses, and patients to ensure safe, cost-effective, and high-quality pharmaceutical care aligned with our organization's standards and regulatory requirements.

*ESSENTIAL DUTIES AND RESPONSIBILITIES*

*Clinical Practice*

* Prepare, compound, and dispense sterile infusion medications in compliance with USP <797>/<800>, state, and federal regulations.
* Review and verify prescriptions, ensuring accurate dosing, compatibility, stability, and appropriateness of therapy.
* Monitor therapy outcomes using lab and clinical data, adjusting regimens as appropriate.
* Provide patient and caregiver education on medication use, administration, and potential side effects.
* Ensure accuracy in infusion device programming when applicable.

*Collaboration & Communication*

* Serve as a key liaison among pharmacy, nursing, physicians, and patients to ensure coordinated and safe care.
* Participate in interdisciplinary team meetings, care planning, and communication to support positive patient outcomes.
* Respond to inquiries from physicians, patients, and healthcare professionals regarding drug use and therapy management.
* Support referral relationships through clear, professional communication and timely service delivery.

*Quality & Compliance*

* Uphold and participate in organizational quality initiatives to maintain The Joint Commission (TJC) accreditation and compliance with all regulatory standards.
* Assist with audits, policy reviews, and documentation readiness activities.
* Ensure all compounding, dispensing, and clinical documentation meets organizational, TJC, and HIPAA requirements.
* Promote continuous quality improvement to enhance patient safety and clinical excellence.

*Digital and Documentation Proficiency*

* Utilize EHR and pharmacy management systems for accurate, real-time clinical documentation and communication.
* Use digital tools for prescription entry, verification, order tracking, and reporting.
* Generate data analyses and reports to assess outcomes and support performance improvement.
* Demonstrate adaptability and commitment to learning and mastering new technology as it is implemented.

*Training, Leadership, and Development*

* Mentor pharmacy staff, technicians, and students in home infusion practice and regulatory compliance.
* Contribute to new staff orientation and ongoing education programs.
* Maintain current knowledge of pharmacotherapy and infusion advancements through continuing education and professional development.

*Administrative and Professional Standards*

* Collaborate with leadership on process improvement and operational goals.
* Provide input for marketing or outreach initiatives as needed.
* Handle customer concerns in accordance with policy, ensuring timely resolution.
* Represent the company professionally, upholding ethical and service standards in all interactions.
* Participate in the on-call rotation and fulfill other duties as assigned.

*QUALIFICATION REQUIREMENTS*

*EDUCATION AND/OR EXPERIENCE*

* Hold a Bachelor of Science in Pharmacy (Doctor of Pharmacy or a Master of Science degree in Pharmacy preferred) obtained from an accredited college of pharmacy.
* Currently hold a valid registered pharmacist license in the state of Ohio.
* Home infusion or sterile compounding experience preferred.
* Strong knowledge of IV admixture preparation, aseptic technique, and infusion devices preferred.
* Capable of assuming responsibility for evaluating and monitoring drug therapy in collaboration with physicians, nurses, and other healthcare professionals to address the total needs and problems of home care patients.

Schedule

* Monday-Friday, pharmacists rotate shifts: 8:00am-4:30pm, 8:30am-5:00pm, and 9:00am-5:30pm
* Pharmacists rotate nightly on-call, weekends, and holidays
