We are in seeking a dedicated & compassionate *Part Time Registered Dietitian (24 hours per week - in person)* to join our interdisciplinary team. The ideal candidate will demonstrate strong leadership skills, critical thinking and a proactive approach to promoting the nutritional wellbeing of our residents.
*Responsibilities include:*
* Complete new admission nutrition assessments and developing individualized care plans
* Conduct annual and significant change assessments to evaluate ongoing nutritional needs
* Document and address weight changes through timely and comprehensive clinical notes
* Perform accurate MDS coding and supporting care plan development with IDT
* Provide high risk nutrition charting
* Collaborate with nursing, therapy and dietary staff to ensure optimal resident outcomes
*Wage: *
* $27.85/hour - $31.85/hour
* Including MI $0.85/hour wage pass through until 09.30.26
