*PT Job Summary:*
The Physical Therapist provides quality care to all residents. The Physical Therapist evaluates and treats pediatric patients, communicates with their families, physicians, and other health team members, maintains all necessary documentation of services in the patient's medical records, and coordinates patient discharges.
*Qualifications:*
* Licensed as a Physical Therapist in the State of California.
* Must have valid CPR certification.
* Must have a completed physical examination from a licensed physician.
* Must have a negative TB skin test within the past year OR a clear Chest X-ray within the past five years.
*Responsibilities and Duties:*
Evaluate patients promptly upon receiving physician referral for treatment.
* Develop effective treatment plans.
* Obtain approval for services from referring physician.
* Treat patients according to treatment plan approved by physician.
* Record evaluations, daily treatment notes, progress notes, and discharge summaries per Company and facility policy and procedure
* Participate in discharge planning.
* Secure necessary durable medical equipment (e.g., orthoses, walkers, standers, etc) for patients to facilitate independent mobility.
* Submit all billing and necessary information on a timely basis.
* Supervise up to two Physical Therapist Assistants.
* Supervise students during the internship.
* Provide clearly defined, career-related (i.e., non-clerical) responsibilities and tasks in accordance with the students' academic programs.
* Inform student interns about ensuring their compliance with company, facility, state, and federal guidelines.
*Other requirements:*
* May be entry level or experienced (willing to train).
* Must have strong computer skills, including proficiency with Microsoft Office.
* Must have excellent interpersonal skills.
* Must exhibit a high level of confidentiality.
