Independent outpatient Dialysis Corporation serving the renal community for over 50 years with locations throughout New York State, currently has positions available for experienced LMSW's to join their interdisciplinary teams.
Candidates should possess the following competencies:
* Screening of referrals for admission
* Outreach to families and community agencies
* Strong assessment skills with formulation of goals
* Excellent written and verbal communication
* Ability to work collaboratively with team members
* Knowledge of eligibilities entitlements and benefits
* The above must be applied with the focus of working with patients and families around issues relating to adjustment to chronic illness.

Pay: $35.00 - $45.00 per hour
