Visiting Physical Therapist (PT) Responsibilities:

* Perform home visits to clients
* Perform diagnostic tests and measurements
* Develop and implement individualized care plans
* Perform continual assessments
* Coordinate with other members of patient's care team
* Provide education and hands-on instruction to patients, their care takers, and family members
* Accurately document observations, interventions, and evaluations in patient's electronic chart

Visiting Physical Therapist (PT) Skills and Qualifications:

* Current NJ Physical Therapist (PT) License
* Reliable transportation and Driver's License
* Strong interpersonal and customer service skills.
* Ability to learn / navigate electronic documentation systems (HCHB or others)
* Bilingual a plus
* Additional training certificates a plus
* New Licensees welcomed

Job Type: Full-time

Pay: $100,000.00 - $110,000.00 per year
