We are seeking a dedicated Outpatient Occupational Therapist to join our healthcare team on a per diem basis, with the potential to transition into a full-time role. This position offers flexibility and the opportunity to work across diverse outpatient settings, providing essential therapy services to a broad patient population. *Duties*

* Assess patients' functional abilities and develop individualized care plans aligned with their goals and medical needs
* Assist patients with Activities of Daily Living (ADLs) to promote independence and safety
* Implement therapeutic interventions across outpatient clinics, home health, schools, or post-acute care settings
* Collaborate with multidisciplinary teams including speech therapists, physical therapists, physicians, and case managers
* Conduct discharge planning to ensure smooth transitions from therapy services to home or community settings
* Manage case documentation accurately and maintain detailed patient records in compliance with healthcare regulations
* Educate patients and families about treatment goals, progress, and strategies for continued improvement
* Participate in ongoing professional development activities to stay current with best practices in occupational therapy

*Requirements*

* Valid occupational therapy license in the state of practice
* Experience working in outpatient clinics; additional experience in inpatient, skilled nursing facilities, home health, or school-based settings is preferred
* Knowledge of assisting with ADLs, discharge planning, and case management processes
* Familiarity with working with diverse populations including pediatrics (early intervention), geriatrics, individuals with autism or disabilities
* Strong understanding of physiology, anatomy, medical terminology, and patient care principles
* Ability to work effectively within acute care environments such as ICU or inpatient units is a plus
* Excellent communication skills for collaborating with healthcare teams and educating patients and families
* Compassionate attitude with a commitment to improving patient outcomes and quality of life
