*Employment Type:* Direct Hire or Contract

*About the Role*
We're seeking a dedicated Physical Therapist to join a skilled nursing/long-term care facility in the scenic community of Libby, Montana. This role focuses on helping residents regain mobility, restore function, and improve overall quality of life through individualized therapy plans.
*Key Responsibilities*
* Develop and implement personalized treatment plans to restore function, improve mobility, and reduce pain.
* Assess patient needs. Set goals. Modify care plans under physician guidance.
* Collaborate with an interdisciplinary team to monitor progress and adjust treatment strategies.
* Educate residents and families on therapy methods, expectations, and progress.
* Document assessments, treatment plans, and progress in accordance with regulatory standards.
* Supervise and support therapy assistants and related staff.
*Required Qualifications*
* Bachelor's, Master's, or Doctorate in Physical Therapy from an accredited program.
* Active Physical Therapist license in Montana.
* Valid CPR certification.
* At least six months of experience in a SNF or LTC setting preferred.
*Preferred Experience & Skills*
* Familiarity with MDS procedures and care planning in skilled nursing environments.
* Strong understanding of HIPAA and applicable federal and state documentation regulations.
* Excellent communication skills with residents, families, and healthcare teams.
* Strong organization skills and a commitment to confidentiality.
