This position is for our coverage and service area from BEDFORD County, Bedford City, Forest, Timberlake and City of Lynchburg, VA .

Job Requirements Include:

* Licensed in the state as a physical therapist assistant (LPTA)
* Graduated from a two year college level program approved by the American Physical Therapy Association, or has two years of appropriate experience as a physical therapy assistant
* Home Health experience preferred but not required

Job Duties/Responsibilities Include:

* Responsible for providing physical therapy under the supervisor of a qualified physical therapist in the home environment
* Participates in educating the patient and family activities of daily living, safety precautions, and necessary modifications in activity, home, or equipment
* Communicates changes to the physical therapist in the patient's status relative to prescribed treatment, and communicates patient status to RN/PT and home care team through individual contact and participation in home care conferences.
* Providing Home Health Physical Therapy services to patients, which includes transfer training, gait training, and therapeutic exercise techniques, etc.
