401(a) retirement account will be established on the employee's behalf. The facility will contribute 3% of the employee’s gross annual pay to a maximum of $245,000 (or other maximum amounts allowed by law). Vesting Schedule applies. In addition to the 401a contribution, employees participating in the 403(b) Voluntary Pre-Tax Retirement Plan will be eligible for a 2% match after meeting eligibility requirements.
Paid Time Off
Paid time off (“PTO”) is available to physician employees to use for vacation, sick days, personal time, holidays, CME, and any other reason for which they require time away from work. Physician employees will receive PTO in accordance with the number of years worked, as listed below:
Less than 5 years: 264 hours of paid time off per calendar year
5 years or more: 288 hours of paid time off per calendar year

Practice overview: Hospital-employed position. Healthcare system, San Luis Valley Health consists of a 49-bed acute care hospital, a 17-bed critical access hospital and three rural health clinics and two multi-specialty clinics.
Type of practice: Outpatient only
Call schedule: Phone call only - 1:9 call and full-time hospitalist program on inpatient side
Expected patient load: 18-22 patients daily average
Procedures needed: General outpatient clinic
EMR – McKesson Practice Partner system.
24/7 hospitalist program
24/7 Respiratory Therapists on staff
Therapy services – outpatient PT, OT, and Speech
Diabetes navigation program along with dieticians on staff
There are currently seven Family Medicine physicians in the healthcare system. Advance Practice Providers are also incorporated into the practice. The system group includes two hospitals and five clinics. Nursing and support staff are provided.

For those patients in need of transfer to a higher level of specialty care, San Luis Valley Health has a relationship with Eagle Air Med, who maintains qualified medical staff locally and a fixed wing aircraft that resides at the Alamosa Airport for expedited air transport. San Luis Valley Health is an affiliate of Centura Health, which helps us increase patient access to a network of readily available resources and specialty services.
