Cleveland Clinic is seeking board certified/ eligible physicians to join our Department of Internal Medicine and Geriatrics. A variety of ambulatory and mixed outpatient-inpatient primary care practice opportunities exist throughout Northeast Ohio at 35 locations: at our Academic main campus, two community academic sites, multi-disciplinary Family Health and Surgery Centers, smaller stand-alone practice locations, and our academic Urban Health Center.
Our practices have achieved medical home certification, use a fully integrated Epic™ electronic health record, and are focused on quality improvement and innovation in care delivery. Team based care with embedded Pharm Ds, Behavioral Health Social Workers, telephonic Primary Care Social Work, RN Care Coordinators, Geriatricians and growing Advanced Practitioner partnerships. Extensive support has been established for population management and value-based care to maximize quality, safety, and efficiency with reduced costs while delivering exceptional patient and team experience. Educational innovation is also prioritized.
