Hartford HealthCare is seeking an Internal Medicine or Geriatric Medicine Physician with experience and interest in providing care in SNF, Sub-Acute, and Long Term Care settings.  Responsibilities include:

  •        SNF/LTC admissions, discharges
  •        follow-up medical management of SNF-subacute residents and LTC residents including visits for change in condition, lab review, care coordination and regulatory visits
  •        attention to transition of care activities along with medication reconciliation
  •        commitment to QA/PI tasks aimed at reducing hospital readmissions. 

This position comes with flexible scheduling options (20 – 30 hours per week), weekday call coverage by our APRN service, and multiple opportunities for professional development in academics, research or leadership in a thriving hospital system.
