Job Summary

 

Essential Duties:

  • Inpatient service responsibilities include hospital consultations, admission history, discharge and daily follow-up as well as medication management.
  • The role will work with the advanced heart failure and mechanical circulatory support inpatient service.
  • Evaluate and manage chronic heart failure patients at all stages of the disease process.
  • Thorough evaluation of patients including their acuity, etiology of presentation, and develop problem-based care plans.
  • Manage stable, chronic heart failure disease or illnesses in outpatient clinic based on program need.
  • Coordinate patient care between specialty physicians, nursing staff and other interdisciplinary providers.
  • Serve as a liaison for the physician in communicating with patients and their families.
  • Hemodynamics and EKG interpretation skills strongly preferred.
  • Provides and coordinates patient teaching and counseling
  • Schedule Monday - Friday, subject to change based on organizational need.

Experience:

  • One year of previous Nurse Practitioner or Physician Assistant experience preferred.
  • BLS required.
  • ACLS required.

License/Certification:

  • WA State license and appropriate certification required.
