In this Medical Director role, you are responsible for front-line clinical engagement around transitional care (TC) goals and objectives, clinical outcomes and strategic decisions in accordance with regional and central clinical leadership. You will be integrated into the core hospital management processes and contribute to action plans created on a regional and site level. Development of physician engagement in the acute setting, post-acute management of preferred provider partners and processes, and clinical oversight of post-acute activities is essential.
Drive Physician Engagement and Process Management

Do you enjoy developing processes that promote increased physician engagement and collaboration with nursing? In this role, you will be helping nurses overcome barriers, develop early ambulation initiatives and improving onboarding of new physicians. In this role, you will develop regional to site level strategies to improve transitional care outcomes focused on the key levers in next site of care, skilled nursing facility length of stay, and readmission.

By partnering with clinical leadership, you will develop high-quality networks and work with post-acute providers on appropriate clinical interventions, protocols and processes to drive down skilled nursing facility length of stay and readmissions. You will support other initiatives in the post-acute care from home health and telemedicine to working with other vendors in the post-acute space with an aim at improving patient outcomes and reducing the cost of care. You’ll develop clear ways to monitor the quality of the transitional care process from site level to regional level.
- We prefer your home base be in the Southeast United States
- 3+ years clinical experience
- Strong interest in transitional care
- Board certified in internal medicine, family practice or geriatric medicine
- Some travel required
