What is PACE?
A Program of All-Inclusive Care for the Elderly, which is designed for frail, 55+ adults, who meet criteria for nursing facility placement, but prefer to live at home and have medical care and supportive services provided. 

Our PACE program
We typically bus our patients in to receive care like rehabilitation and check ups, but also provide social events, games and a sense of community. Our PACE program in one of 11 in the state of Virginia and one of 100+ nationwide.

A minimum of one year experience working with the elderly population, chronic disease management, long-term care or disabilities.

Requires: MD or DO from an accredited medical school and board certification or eligibility in Internal or Family Medicine; Board certification in Geriatric Medicine is preferred.

Centra PACE – Farmville

The Centra PACE mission is to enhance life for older adults with chronic needs by providing care and services from a team of compassionate, qualified professionals. PACE strives to achieve this mission by empowering participants to live in their homes and in the community for as long as it is medically and socially safe.

What is PACE?

PACE is national program of  comprehensive care for adults 55 years of age and older who meet the criteria for nursing facility placement, prefer to stay at home and have an assessment indicating that living at home with the support of the PACE program is a safe alternative.  PACE promotes independence and the highest levels of functioning while allowing choice and dignity for members and their families.

Comprehensive services are delivered by a team of professionals made up of a primary care physician, registered nurses, a masters level social worker, health aides, transportation personnel, rehabilitation therapists, dietitians and recreation/activity personnel. The team works with participants and their families to develop a tailored plan of care which is monitored closely and updated as needed.

PACE participants can be picked up from their home each morning and brought to the PACE Center. They participate in social activities, eat a hot lunch with friends, and receive medical assistance as needed from a dedicated, compassionate care team. At the end of the day, the PACE van takes the participants back home to enjoy family, friends and the surroundings they know and love.

Centra PACE provides a range of services twenty-four (24) hours a day, seven (7) days a week, three hundred sixty-five (365) days a year. Primary medical care from doctors, nurses and therapists is provided at the Centra PACE Center Monday through Friday 7 a.m. to 5:30 p.m.
