Key Responsibilities

Careline Physicians Services provide primary and/or specialty medical care in a variety of Post-Acute Care setting such as Assisted Living, Short Term Rehab and Long-Term Care. The Physician is responsible for providing medical leadership to non-physician practitioners such as Nurse Practitioners and Physician Assistants. The Physician will function in collaboration with providers according to a Collaboration Agreement. The Physician’s practice will focus on caring for the elderly and frail population while providing advanced primary care to assist patients and families with comfort and quality of life.

In this role you will be responsible for:

  • Providing medical evaluations for elderly, frail, and seriously ill patients.
  • Performing under stress and working collaboratively in a team atmosphere.
  • Ability to drive between locations.
  • Communicating significant patient findings, problems, and changes in condition or environment to the clinical coordinator, family, facility, and other personnel involved with your patient care.
  • Routinely assess your patient’s status by obtaining relevant health and medical history. Perform an exam based upon the age and patient history. Identify significant health or medical risk factors to ­­incorporate into a comprehensive Plan of Care.
  • Order, interpret, and evaluate laboratory or consult findings then incorporate alteration in treatment into the Patients Plan of Care.
  • Perform evidence based diagnostic and/or therapeutic interventions consistent with certification, law and state medical privileges. Ability to prescribe and monitor medications as allowed by prescriptive authority through the appropriate State Board Agency.
  • Develop a treatment plan in collaboration with the patient, family, facility and Careline support staff. Collaborates with nursing, medical and other healthcare team members to achieve the highest possible patient outcome.
  • Refers patients to specialist as appropriate for consultation or health resources and treatment.
  • Assures continuity of care by serving as a liaison between patient, family, and other members of the multidisciplinary care team.
  • Assist in clinical oversight of nurse practitioners and physician assistants per the Collaborative Agreement.

Required Qualifications

  • Doctor of Medicine or Osteopathy required
  • Current unrestricted State license to practice medicine in Michigan
  • Requires demonstrated interpersonal, supervisory, and management skills with ability to communicate effectively and assist in lending multi-disciplinary team to achieve highest possible patient outcomes
  • Demonstrates time management skills and adaptability to the evolving healthcare environment
  • Exhibits working knowledge of both medical regulations and reimbursement
  • Sit, stand, bend, lift, and move intermittently
  • Must be able to lift at least 35lbs.
  • Current and valid Michigan driver’s license
  • Reliable transportation and verifiable automobile insurance coverage

Desired Qualifications

  • Board certification in Family or Internal Medicine, Geriatric, Palliative/Hospice experience preferred.
  • Geriatric, Palliative/Hospice, Post-Acute Care (Nursing Home/Assisted Living) experience preferred but not required.

Hours

  • Administrative Office hours: 8:00 AM – 5:00 PM, Monday through Friday
  • On Call Rotation
