We seek a physician with a passion for primary care excellence to guide the vision for our dynamic group of primary care physicians and providers, in partnership with site-based Medical Directors and SHMG Leadership. The Network Medical Director will maintain a clinical practice with a reduced patient panel and will dedicate the remainder of time to leadership activities.

SHMG Primary Care Network Highlights:

• Nine community-based geographically dispersed primary care practices
• Practices have earned National Committee for Quality Assurance (NCQA) recognition, each certified as a Level 3 Patient-Centered Medical Home.

As Medical Director, serve as the clinical leader and participate in operational financial performance, strategic planning, recruitment, mentorship, retention and provider oversight, including 360-degree provider evaluations. Create an environment to enhance physician/provider and staff engagement and clinical integration, with responsibility to drive key population health goals: access to care, quality, utilization within primary care.

Lead Primary Care transition from a fee-for-service to capitated payment model, working with administrative and clinical team members to design the care delivery approach to achieve optimal patient outcomes and ensure long-term viability and sustainability. Collaborate with Hospital leadership on choosing payer quality/incentive initiatives.

Provide leadership to support the practice and organizational goals to include, but not limited to:

• Quality improvement
• Patient satisfaction
• Fiscal responsibility
• Provider Engagement

Requirements: Board Certified MD or DO (Family Medicine, Internal Medicine)

• Experience in clinical transformation: developing and implementing quality programs, care pathways, clinical guidelines and best clinical and operational practices in primary care.

• Exemplary leadership qualities and clinical expertise in the execution of our clinical model of care, understands the underlying business model and provides input into administrative policy development and decision-making processes.

• Flexible team player, with exceptional communication skills – able to collaborate well with diverse set of team members.

• Participate and promote clinical and interdisciplinary team member collaboration to meet common goals.

• Five to seven years of progressive leadership advancement within primary care.

The Albany Med Health System

If interested, our physicians have the opportunity to have medical students rotate through their practice. Surgical Residents and Emergency Medicine Residents rotate from Albany Med with Saratoga Hospital Medical Group - General and Minimally Invasive Surgery and Saratoga Hospital Medical Group - Emergency Medicine in the Emergency Department. Saratoga Hospital's first Family Medicine Residency class will begin Summer 2023. Physician Assistants, Nurse Practitioner and CRNAs from regional programs also rotate through some of our practices. 

The hospital is experiencing overall growth to meet the needs of the community. Since 2000, major construction projects include:

• Whitney-Hendrickson CardioVascular Interventional Suite offers cardiac catheterizations and interventions with a 24/hour STEMI receiving program, peripheral vascular angiography and  interventions. The suite is utilized by Cardiology, Interventional Radiology and Vascular Surgery. (2000, Recently renovated with second interventional suite added with pre and post-op rooms.)
• Saratoga Surgery Center: Saratoga Surgery Center offers six ORs, one procedure room, and two treatment rooms. (Constructed in 2004) 
• Alfred Z. Solomon Emergency Center: In 2009, the Emergency Department was constructed, with 36, state-of-the-art private rooms. The Department remains in like-new condition. 
• Mollie Wilmot Radiation Oncology Center: Constructed in 2005, was recently renovated with a new addition. The $6 million project included a second linear accelerator.
• Marylou Whitney and Desmond DelGiacco, MD, Intensive Care Unit: 19-bed, state-of-the art Intensive Care Unit opened in 2016.
• The Surgical Pavilion at Saratoga Hospital: twelve operating rooms, supporting minimally invasive surgery, endoscopy suite renovations, pre-op suite, PACU, registration and waiting areas, was complete in 2016.

Reaching Out

Wilton Medical Arts, located three miles from the hospital, offers medical imaging: (3 tesla wide bore MRI; CT, PET/CT; complete breast health center with breast MRI and on-site biopsies; DEXA, ultrasound, and x-ray); Laboratory, Urgent Care, and the Saratoga Hospital Center for Breast Care.

The hospital also offers primary care at nine locations around the region: Saratoga Springs, Scotia Glenville, Schuylerville, Galway, Malta, Mechanicville and Milton. The Saratoga Community Health Center offers primary care, mental health and dental services in the heart of Saratoga, serving the underserved.

Saratoga Surgery Center is located on the Wilton campus with physician practices conveniently located on the second floor, including: Allergy and Immunology, Pediatric Ophthalmology, Ophthalmology, Otolaryngology, and Interventional Pain Management. We also offer medical nutrition therapy in this space.

The Regional Therapy Center offers services in a variety of settings at five locations. Our Wilton site offers PT, OT, athletic training, performing artist rehab; a lymphedema clinic and incontinence therapy. Washington Street is located just blocks from the hospital nearby building where PT, OT, Speech therapies are offered along with specialty services including: aquatic PT, hand therapy, and vestibular rehabilitation, are offered. 6 Care Lane offers PT, OT, cardio-pulmonary rehab. Our new Malta location is described below. Milton offers PT and vestibular rehab.

In collaboration with Albany Medical Center, Malta Med Emergent Care, located on Saratoga Hospital's 140 acre southern medical campus, offers 24-hour emergent care staffed by board certified emergency physicians, with advanced imaging (MRI, CT, US and x-ray) and laboratory services on the first floor and on the second floor medical offices, specialty services, an additional imaging suite offering mammography and DEXA, and infusion center.

Saratoga Hospital and The Saratoga Regional YMCA opened a 55,000 square foot facility on the Malta campus in 2018,  a shared facility that is the home for hospital-affiliated medical and specialty offices and the Y’s Malta Branch. This Y is the first of its kind in the area and will incorporate traditional Y health and wellness activities and programs, as well as a unique level of design, which creates an integrated facility that collectively serves the needs of individuals in the community. Our medical offices at this space include: primary care, sports medicine, specialty physician suite in addition to the Regional Therapy Center offering PT, OT, Speech, including specialty programs of Vestibular Rehab, Pelvic Floor and Aquatic Therapy program.

A third building opened in 2021, The Bone and Joint Surgery Center at Exit 12, a joint venture with Albany Medical Center, Saratoga Hospital and Capital Region Orthopedic Associates.

The medical park is located close to Luther Forest Technology Campus where the GlobalFoundries ^TM is located. The $ 4.2 billion chip factory manufactures computer chips from silicon wafers which has brought hundreds of families to the region.
