District Medical Group Children’s Rehabilitative Services (DMG CRS) offers families a state-of-the-art facility with a friendly, professional environment. DMG CRS is uniquely designed to meet the health care requirements of special needs children by offering nearly all specialty and primary care as well as support services in a single location.

DMG CRS includes more than 85 medical providers across more than 25 medical specialties, serving children from birth to age 21. Providing coordinated, integrated care, DMG CRS primary care and therapy providers work directly with specialty physicians to deliver comprehensive treatment, utilizing one electronic medical record (EMR). In addition, service teams such as nutrition and diet, child life and social work collaborate with our renowned team of pediatric medical professionals to provide vital support to your child and family.

Some of the medical specialties and services offered at DMG CRS include: primary care, dental health, physical and occupational therapy, orthopedics, speech, audiology, plastic surgery, cardiology, neurology and behavioral health. Some conditions covered include but are not limited to: cleft lip and palate, muscular dystrophy, spina bifida and cerebral palsy. DMG CRS is located conveniently in Central Phoenix at Park Central Mall, which is on 3^rd Avenue between Thomas and Osborn Roads. Our location is one-story for easy access and includes free parking right outside the main entrance.

DMG CRS is the largest Multi-Specialty Interdisciplinary Clinic (MSIC) in the state of Arizona and the only MSIC in Maricopa County. DMG CRS accepts AHCCCS Complete Care health plans, most private insurances, and private-pay patients.  DMG CRS is licensed by the Arizona Department of Health Services (ADHS).
