coordinated health care in the home. This includes HarmonyCares Medical Group,
HarmonyCares Home Health, HarmonyCares Hospice and In Home
Assessments.
The Medical Group focuses on providing primary care to high acuity geriatric patients in their home setting with a Medical Assistant driving the company car. We schedule patients Monday through Friday during the day hours, we do not work weekends or holidays.
