Bluestone Physician Services is a mobile primary care practice offering a unique and robust model of care; customizing care that is preventive, proactive, and includes all members of the care team, patients and their families.  Join us in our team-based care as a Nurse Practitioner or Physician Assistant as we deliver best-in-class primary care to those with intellectual and physical disabilities.  Our providers have deep knowledge of the unique aspects of Group Home settings of care. This role will serve patients that reside in group homes in the east and northeast area of Minneapolis/St. Paul such as Maplewood and White Bear Lake.

Our Care Model delivers customized care that is preventive, proactive, and includes all members of the care team, patients and their families.  Our patients experience 21% less ER visits, 36% less hospitalization and 41% less hospital re-admissions compared to patients with similar conditions and complexities.
