Job Description:
Case Manager RN

The Case Manager is responsible for facilitating the delivery of care and services to the Emergency Department patient and through the Admitting Department. Applies approved admission criteria and intervenes when required to prevent unnecessary inpatient admissions. Facilitates the provision of medically appropriate, cost effective care by identifying and referring issues and problems to the appropriate department leader or administrative representative.

  • Works effectively under stress and frequently changing job requirements/situations
  • Maintains regular, consistent and punctual attendance at the assigned job location
  • Effective, positive verbal and written communication skills
  • Able to read, write legibly, and converse effectively in the English language
  • Responds effectively to patient crisis/emergency situations
  • Demonstrates competence in use of InterQual
  • Proficient in use of telephone, computer, including accessing internet, word processing, and documenting in an electronic medical records system
  • Demonstrates competence in knowledge of Medicare/CMS guidelines as they apply to case management

  • Licensed to practice as a Registered Nurse in the state of Oregon
  • Certificate in Case Management preferred
  • American Heart Association Healthcare Provider Basic Life Support (BLS)
  • Bachelor's degree in Nursing preferred

  • At least seven years of experience in discharge planning activities in an acute care hospital
  • Five (5) years of clinical experience as a registered nurse
  • Prior experience in an acute care emergency department preferred
  • Prior experience in behavioral health preferred
