*Duties*
* Conduct comprehensive evaluations of patients to assess their physical abilities and limitations.
* Develop and implement tailored treatment plans based on individual patient needs.
* Utilize various therapeutic techniques, including trigger point therapy, to promote recovery.
* Collaborate with multidisciplinary teams to coordinate patient care and optimize treatment outcomes.
* Educate patients and their families on exercises, care plans, and strategies for managing their conditions.
* Monitor patient progress and adjust treatment plans as necessary to ensure optimal recovery.
* Participate in discharge planning to facilitate a smooth transition from therapy to home or other care settings.
* Maintain accurate documentation of patient progress and treatment interventions in compliance with clinic policies.
*Requirements*
* Current state licensure as a Physical Therapist.
* Experience in outpatient, inpatient, pediatrics, or home health settings is preferred.
* Knowledge of systems related to physical therapy practices and patient management.
* Strong communication skills for effective interaction with patients, families, and healthcare professionals.
* Ability to develop comprehensive care plans tailored to individual patient needs.
* Familiarity with discharge planning processes to ensure continuity of care post-treatment.
* Commitment to ongoing professional development and staying current with industry best practices.
