We are a family-owned company seeking a qualified Licensed Practical Nurse (LPN). With the support of our health services team, this position will oversee the clinical program for our specific needs provider location. This is an exciting opportunity for an LPN looking to expand his/her clinical and leadership skills in a rapidly growing industry. Salary of $80,000 plus benefits including health insurance, 401k and unlimited PTO. Other unique benefits for this LPN position is being able to work regular hours M-F.

*Job Summary*
We are seeking a qualified and compassionate LPN to lead our clinical team and oversee all clinical operations. The ideal candidate will have experience in clinical assessments, changes of condition, medication and pain management and quality assurance. Experience in residential care is helpful and desirable but not required. As the Wellness Director, this position is responsible for ensuring that clinical services are provided in a safe and caring manner while maintaining compliance with state and federal regulations.

*Responsibilities*

* Provide direction and guidance to the clinical and interdisciplinary team
* Identify patient changes of condition and development care plans timely and accurately
* Lead care conferences with families and the interdisciplinary care team
* Ensure effective communication between the care team, patients, family, physicians and case workers
* Ensure changes in condition are evaluated, monitored, documented with effective interventions implemented
* Perform skilled nursing tasks as defined by provider policies and procedures and state law
* Oversee medication management systems to ensure medication is administered in a safe and effective manner
* Effective monitoring of patient health concerns pertaining to weight and nutrition, skin integrity, mobility, and pain management
* Monitor and conduct proper documentation for medical record compliance and quality assurance
* Serves as a leader and role model for other team members in demonstration of professional, responsible and compassionate care

*Requirements*

* Licensed Nurse in good standing with State Licensing authority
* Experience in training and team member development preferred
* Must be able to read, comprehend, write, and speak English to communicate effectively with patients, families and providers
* Effective documentation skills
* Must have basic computer skills and ability to learn Electronic Medical Records
* Ability to follow systems for monitoring health conditions and for ensuring efficient and safe delivery of care
* Strong organizational skills and ability to prioritize daily tasks
* Must maintain current CPR/First Aid credentials
* Must be able to perform duties and responsibilities (essential functions) with or without accommodation
* Must have acceptable references
* Subject to pre-employment screening including criminal background check, illegal drugs, TB testing and physical
