*IMMEDIATE NEED:* PT needed for home health visits in Houston, Cypress, Katy, and surrounding areas on a full time basis . *THIS IS ONE OF THE BEST THERAPY JOBS YOU WILL EXPERIENCE DURING YOUR CAREER.*

Preferred Experience:

*Kinnser/Oasis* experience preferred with at least *one year in home health* and strong clinical skills. *PT Evaluations, Re-Evaluation, PT OASIS SOC/Recerts/DC , and PT visits* for mostly ortho and some medically acute patients. *Strong analytical/critical thinking skills needed.* Fast-paced environment which makes the day fly by. Strong, collaborative nursing team that really cares for one another and will make your job both rewarding and FUN!

Work Hours: 30 Units/week

Job functions Include but not limited to:

- Identify and meet patients' goals and needs

- Offer cost-effective treatments that help improve clients' motion, mobility, and endurance

- Coordinate care with other disciplines to ensure positive outcomes for the patient

- Provide excellent patient care

- Develop care plans using a variety of treatment techniques

- Create fitness- and wellness-oriented programs tailored to patients' specific needs

- Provide quality, personalized and evidence-based care and proven interventions

- Motivate patients during treatment in order to help them function optimally

- Promote clients' healthy lifestyle by improving strength, flexibility, balance, and coordination

- Consult and practice with other health professionals

- Evaluate effects and monitor and communicate progress

- Document patient care services

- Supervise PTA's, if needed

- Provide coordination of care between multiple disciplines in the home
