Purpose: Licensed Clinical Social Worker (LCSW) or Licensed Professional Clinical Counselor (LPCC) shall perform clinical duties that include assessment, initial diagnosis, treatment team collaboration and clinical treatment. The independent clinical counselor will see patients in individual, group or family sessions for the treatment of substance abuse or mental health.

*Principal Responsibilities: *

1. Utilizing knowledge of a variety of therapeutic models and theories to understand and plan intervention strategies for a variety of patients.

2. LCSW/LPCC agrees to use only evidenced-based techniques that are proven to be effective and safe for patients seeking care.

3. Attend trainings, as scheduled, for evidenced-based trainings including, but not limited to, CBT, TFCBT, DBT, EMDR, MRT, ACE and others.

4. Monitoring patient progress and making changes to the therapeutic approach and best practice, evidenced based interventions based on patient response to treatment.

5. Working collaboratively with the patient and treatment team to develop a comprehensive care plan that addresses priority needs of the patient, desired treatment outcomes, an agreed upon plan of action, reassessment and more.

6. Maintaining patient records related to screening, intake, treatment, continuing care, progress toward goals and objectives, discharge summary, and treatment outcomes that are accurate, concise, timely, and useful for coordinating treatment team planning, monitoring, and review.

7. See patients in a variety of settings including individual, group and family counseling, assessment and screening and in crisis situations.

8. Participate in supervision and team meetings.

9. Participate in court hearing at the request of the individual patient or court order.

10. Participate in monitoring of individual satisfaction with supports, services, and programs.

11. Maintain a working knowledge of current trends and developments in the field.

12. Maintain compliance with Dept. for Behavioral Health, Developmental and Intellectual Disabilities, and agency Policies and Procedures.

13. Maintain and submit all required records, documents, and reports in a timely manner.

14. All other duties as assigned.
