*Job Summary*
The Physical Therapist is responsible for assessing, planning, implementing, and evaluating therapy services for patients across the care continuum. This role adheres to the Physical Therapy Practice Act and provides individualized treatment through safe, effective, and evidence-based interventions.
*Responsibilities*
* Collect and review patient data from charts and care team members; collaborate with physicians and other providers as needed.
* Perform independent assessments for patients across a wide range of conditions in inpatient and outpatient settings.
* Develop and implement individualized treatment plans with measurable, attainable goals.
* Reassess and monitor patient progress regularly in accordance with departmental and regulatory standards.
* Educate patients and families on therapy techniques, home programs, and equipment use.
* Collaborate with interdisciplinary teams to ensure continuity of care and optimal patient outcomes.
*Requirements*
* Doctor of Physical Therapy (DPT) degree
* Current Physical Therapy (LPT) license in the state of Texas
* Basic Life Support (BLS) certification
