*Position Summary *The Physical Therapist provides skilled therapy services to patients in their homes. This includes evaluating patient needs, developing individualized treatment plans, delivering therapeutic interventions, and educating patients and families. The PT works closely with the interdisciplinary team to achieve the best possible outcomes. We're seeking a *part-time Physical Therapist (PT)* to provide in-home therapy to patients in Southern Indiana. This is a great opportunity if you're looking for flexible hours, supplemental income, or the chance to be part of a growing agency where your input truly matters. *Key Responsibilities *Conduct initial and ongoing physical therapy evaluations in the home setting. Develop and implement individualized treatment plans to restore mobility, strength, balance, and independence. Educate patients, families, and caregivers on exercises, safety, and home modifications. Document visits, progress, and outcomes in accordance with agency policies and regulatory requirements. Collaborate with nurses, occupational therapists, and other team members to coordinate care. Participate in care conferences and contribute to patient-centered care planning. Ensure services are delivered in compliance with Medicare/Medicaid, state, and agency standards.
*Qualifications *Current, active Physical Therapist (PT) license in the State of [State]. Minimum of 1 year of PT experience (home health experience preferred, but not required). Strong clinical assessment, communication, and documentation skills. Self-motivated, organized, and comfortable working independently. Reliable transportation and ability to travel within service area. CPR certification (current).
