*Paradigm Home Health is 5 star Home health agency, owned by a physical therapist, with 9 locations across TX, and is seeking a full-time Physical Therapist who will provide direct patient care to home bound patients in the Temple or Waco areas. Company Car provided\**

As a therapist in our organization you will have the following key responsibilities:
- Develop and revise plan of care in consultation with physician and other care-team members.
- Provide therapeutic treatments and evaluation of equipment needs as applicable.
- Participate in clinical conferences and in-service programs.
- Conduct on-site supervisory visits of physical therapy assistants.
- Advise and consult with the family and other agency personnel as applicable.
- Prepare clinical and progress notes.
- Protect and honor patient and coworker confidentiality and right to privacy.
- Evaluate outcomes of care.
- Plan for discharge.
We are looking for licensed, team oriented therapists, with excellent clinical and interpersonal skills, that have the ability to work independently in the field.
Qualifications include the following:\*
*- Current Texas License as a Physical Therapist.*
*- Current Certification in CPR*
*- Valid Texas Drivers License and Insurance, with reliable transportation*
*- Must have a minimum of 1 yr prior home health experience, within the past 3 yrs.*
\*Knowledge of Kinnser Software documentation preferred
