*Physical Therapist (PT) PRN*
*Position Overview*
We are looking for a dedicated Physical Therapist to lead and provide high-quality physical therapy services for our residents. In this role, you'll help residents improve mobility, strength, and independence while ensuring all therapy practices meet state, federal, and facility standards. You'll collaborate with an interdisciplinary team and play a key role in care planning, assessments, and resident outcomes.
*Key Responsibilities*
* Provide evaluations and individualized physical therapy treatments to residents.
* Lead and oversee the facility's physical therapy programs and daily operations.
* Participate in care planning, MDS assessments, and discharge planning.
* Maintain accurate documentation, including treatment notes and progress records.
* Collaborate with nursing, medical staff, and other departments to coordinate therapy services.
* Educate residents, families, and staff on therapy techniques, equipment use, and safety.
* Support continuous quality improvement efforts within the therapy department.
* Ensure compliance with resident rights, safety standards, and confidentiality (including PHI).
*What We're Looking For*
* Licensed Physical Therapist (PT) in the state of Wisconsin.
* Strong clinical judgment, communication, and leadership skills.
* Commitment to compassionate, resident-centered care.
Job Types: Part-time, PRN
Pay: $45.00 - $65.00 per hour
Expected hours: 1 - 16 per week
