Riverview Life Skills Center is a privately owned 11 bed Enhanced Residential Community Care Home/ Choices For Care provider. Our mission and purpose is to provide our residents with the resources (PCP, Neurologist, OT, PT, SLP and counseling) to rebuild their lives and to provide independent living skills training/ rehab. We assist our residents in reaching their highest potential, physically, emotionally and cognitively. Riverview is a Licensed Level 3 Community Care home providing rehabilitation and also a long term care facility for TBI survivors.
RN Duties and Responsibilities:
Provide exceptional resident care by assessing, planning, implementing and evaluating client care plans
Monitor residents vitals; detect changes in symptoms, health, and/or pain and report any significant changes to the clients health care team (PCP).
Administer medications and treatments as prescribed by PCP and or Neurology ensuring compliance and provide instruction and oversight to all direct care personnel regarding each resident's health care needs and nutritional needs.
Maintain a safe and clean working environment by complying with Vermont Residential Care/Assisted Living procedures, rules per residential care home and assisted living residence licensing regulations.
Complete and update patient documents, charts, Medication Administration Registry, nursing notes and care plans.
Manage Client medication along with PCP, Neurologist and Health Direct Pharmacy.
Follow all aspects of RN role for a community care home as stated by Vermont Residential Care/Assisted Living Survey Licensing Regulations.
Will build and train staff member team. Will delegate various care giving duties to trained staff members.
Will annually provide 12 hours of training to staff as stated by the VT Residential Care/Assisted Living Survey Licensing Regulations.
Will provide constructive training/criticism and support to all staff when needed and and will keep staff moral at the highest possible level by providing an inviting, caring and supportive work environment.
