*Position Summary: *
The Contract Physical Therapist (PT) provides direct physical therapy services to residents in Oak Hills Manor, LLC, a California Congregate Living Health Facility (CLHF), in compliance with the standards set forth by Title 22, California Code of Regulations. This role involves evaluating, treating, and developing rehabilitation plans to improve mobility, strength, and functionality for residents with various medical conditions. The PT collaborates with the interdisciplinary care team to ensure the delivery of high-quality care in a safe and supportive environment.
*KEY RESPONSIBILITIES:*
• *Assessment & Evaluation*
o Conduct initial and ongoing assessments of residents' physical status, mobility, and functional abilities
o Use evidence-based evaluation techniques to determine the need for physical therapy and develop individualized treatment plans
o Assess progress and adjust therapy goals based on clinical outcomes and feedback from residents and the interdisciplinary team
• *Treatment & Rehabilitation*
o Design and implement therapeutic interventions to restore, maintain, or prevent decline in physical function
o Provide direct physical therapy services, including exercises, modalities, manual therapy, and assistive device recommendations
o Educate residents and caregivers on proper body mechanics, safe movement techniques, and home exercise programs
• *Documentation & Compliance*
o Maintain accurate, timely, and comprehensive documentation of all assessments, treatments, and progress notes in accordance with *Title 22 *regulations and facility policies
o Ensure that therapy interventions and progress meet or exceed California Department of Public Health (CDPH) standards for CLHFs
o Comply with all local, state, and federal regulations regarding patient confidentiality (HIPAA) and patient safety
• *Collaboration & Communication*
o Work closely with the interdisciplinary team, including physicians, nurses, occupational therapists, and social workers, to develop and implement care plans
o Communicate effectively with residents, family members, and other stakeholders regarding physical therapy interventions and progress
o Participate in team meetings, care plan reviews, and case conferences as necessary
• *Supervision & Education*
o Provide guidance and mentorship to rehabilitation aides or other support staff (if applicable)
o Educate staff on physical therapy principles, equipment, and techniques to improve overall care and outcomes
• Exercise discretion and independent judgement that is free from immediate supervision in relation to items with significant impact on the business

*QUALIFICATIONS:*
• *Education:*
o Doctor of Physical Therapy (DPT) or Master's in Physical Therapy (MPT) from an accredited program
o Active Physical Therapy license in the state of California
• *Experience:*
o Previous experience working in a congregate living health facility, skilled nursing facility, or similar healthcare setting preferred
o Experience with geriatrics, post-acute care, or individuals with chronic conditions is highly desirable
• *Skills & Abilities:*
o Strong knowledge of physical therapy interventions, assessments, and equipment
o Ability to work independently and collaboratively in a multidisciplinary team
o Excellent communication skills with residents, families, and healthcare team members
o Ability to comply with Title 22 regulations and maintain accurate medical records
• *Certifications:*
o Current CPR and First Aid certification (or the ability to obtain upon hire)

o Ability to lift, move, or reposition patients, in accordance with facility policies
o Flexible and adaptable to the needs of a dynamic healthcare environment
*PHYSICAL DEMANDS& WORK ENVIRONMENT:*
• This position requires standing, walking, lifting, and occasional bending
• Must be able to move patients safely and effectively, following safety protocols
• Work is performed in a healthcare setting with exposure to potentially hazardous materials, including bodily fluids, medical equipment, and medications
Job Type: Contract
Pay: $90,000.00 - $138,000.00 per year
