Progressive Care Therapy is looking for a *Physical Therapist* to join our team. We are a locally owned, private practice clinic based out of Grand Rapids, MN. Our private practice setting offers our therapists control over their *own schedule*, an *extremely manageable caseload*, and the ability to have *flexible* hours to support a healthy work-life balance.
We're looking for someone as equally dedicated to the delivery of effective, evidence-based clinical practice with a focus on outpatient therapy and home health services. Our clinic offers warm water aquatic therapy services to adjunct traditional land-based approach to any ailment. We are an educational clinic that stresses the use of evidence-based practice and offers up to $1,200 a year in continuing education dollars.

*Responsibilities*
• Develop a written plan of care specific to each patient and implements therapy services based on that plan of care.
• Initiates physical therapy program and instructs other personnel in certain phases of physical therapy with which they may work with a patient, as well as instructing them as to the goals of the physical therapy program for the patient by participating in case conferences.
• Prepares and submits clinical and progress summaries based on the attainment of goals.
• Participates in peer consultation process.
• Supervises physical therapy assistants according to organization policy and state regulations.
• In this position you will help to develop a therapy program for both outpatient therapy and home health patients.
