We are currently looking for a compassionate, organized, and team-oriented individual to fill the role of Full-Time Occupational Therapist (OT) position for the city and county of San Francisco.

Responsibilities:
The occupational Therapist will provide services within the scope of occupational therapy, following the plan of care individualized for the patient, and coordinating with the Interdisciplinary Team
The Occupational Therapist will follow the plan of care; provide treatment; evaluate the patient's reaction to treatment and report any changes in his/her condition to the Physician, Case Manager, and/or Director of Nursing
Maintain appropriate clinical records and/or notes and submit the records with organization policy.
Be responsible for following all company policies and procedures
Pay Rate:
$60.00 - $72.00 per hour DOE
$10,000 Sign on Bonus

Requirements
Current OT License to practice in California - Required
Home Health: 1 year - Preferred
Current CA Driver's License
Posses Excellent Organizational Skills
Able to travel within a 15 to 25-mile radius on a regular basis
