We're seeking a *Registered Nurse Case Manager* who is passionate about making a difference. As a key part of our interdisciplinary team, you'll oversee and coordinate the care of hospice patients, ensuring they receive the highest level of clinical and emotional support.
*What You'll Do*
* Manage a caseload of hospice patients, providing comprehensive nursing care
* Develop and implement individualized care plans
* Communicate closely with patients, families, and the hospice team to ensure comfort and understanding
* Provide education and emotional support to families during end-of-life care
* Ensure documentation meets compliance and quality standards
*What We're Looking For*
* *Active RN license* in Pennsylvania
* *Hospice or home health experience* preferred (but we'll train the right nurse!)
* Strong communication and organizational skills
* Compassionate, patient-focused mindset
* Valid driver's license and reliable transportation
