*About Us:*Harmony Hearts Recuperative Care provides compassionate recuperative care for individuals experiencing homelessness or medical vulnerability after hospital discharge. We help clients transition safely to recovery in a supportive environment while connecting them to long-term community resources.
*Position Summary:*We are seeking a dedicated and detail-oriented Social Worker to join our Recuperative Care team. The Social Worker will manage client intake, complete necessary paperwork, and coordinate services to ensure a smooth recovery process. This role requires strong interpersonal skills, clinical knowledge, and the ability to work independently while supporting clients with diverse needs.
*Key Responsibilities:*
* Conduct comprehensive client intakes, including assessment of medical, social, and environmental needs.
* Complete all required documentation, including intake forms, service plans, consent forms, and referral paperwork.
* Develop individualized care plans in collaboration with clients, healthcare providers, and multidisciplinary teams.
* Provide case management services, including monitoring client progress and adjusting care plans as needed.
* Connect clients to community resources such as housing, healthcare, mental health services, and financial assistance programs.
* Advocate for clients and ensure their rights and dignity are upheld.
* Maintain accurate and timely records in compliance with organizational policies and regulatory standards.
* Participate in team meetings, trainings, and quality improvement initiatives.
* Respond to crises and provide supportive counseling as needed.
*Qualifications:*
* Bachelor's or Master's degree in Social Work (BSW/MSW) from an accredited program.
* Current license or eligibility for licensure as a Social Worker in Minnesota.
* Minimum 3-4 years of experience in social work, preferably in healthcare, home care, or supportive housing settings.
* Strong knowledge of community resources and social services systems.
* Excellent communication, documentation, and organizational skills.
* Ability to work independently, prioritize tasks, and manage multiple cases simultaneously.
* Compassion, cultural competence, and sensitivity to diverse populations.
*Preferred Qualifications:*
* Experience working with medically fragile, homeless, or vulnerable populations.
* Knowledge of recuperative care or transitional care programs.
* Familiarity with electronic health records (EHR) and case management software.
*Physical Requirements:*
* Ability to show up at healthcare facilities, or community resources as needed.
* Ability to assist clients as required.
