*Position Summary:*
Mind Above Matter is seeking a dedicated *Primary Geriatric Therapist* to provide group and individual therapy to adults in a nursing home setting as part of our *PHP/IOP programming. *This role is central to improving the emotional well-being and quality of life for geriatric clients by facilitating mental health interventions tailored to their unique needs.
*Key Responsibilities:*
* Provide individual, group, and family therapy to adults in accordance with PHP/IOP program standards.
* Develop, implement, and document individualized treatment plans in collaboration with an interdisciplinary treatment team.
* Maintain compliance with state regulations and professional licensing standards.
* Participate in case reviews and clinical meetings to ensure comprehensive care coordination.
* Engage residents in meaningful therapeutic activities and foster a supportive group environment.
* Complete timely and accurate documentation in the electronic health record (EHR) system.
*Qualifications:*
* Master's degree in Social Work, Counseling, Marriage and Family Therapy, or a related mental health field.
* Active licensure in Texas: LCSW, LMSW, LPC, LPC-A, LMFT, or LMFT-A.
* Preferred: Experience working in PHP/IOP settings or with geriatric populations.
* Strong interpersonal and communication skills.
* Familiarity with PC usage and electronic charting (EHR experience preferred).
