A long-standing hospice organization serving patients across New York City is seeking a Medical Social Worker to support individuals and families facing advanced or terminal illness. This role focuses on providing emotional, psychosocial, and bereavement support as part of an interdisciplinary care team dedicated to comfort, dignity, and quality of life.
*Key Responsibilities*
* Provide psychosocial assessments for patients and families receiving hospice services
* Assist individuals and caregivers in coping with terminal illness, crisis situations, and changes in family dynamics
* Participate in care planning with an interdisciplinary team and adjust interventions as patient needs evolve
* Offer short-term, goal-focused counseling and bereavement support
* Assess financial and social needs, and connect families with appropriate community resources
* Maintain detailed and timely documentation, including assessments, progress notes, and interventions
* Consult with hospice staff, volunteers, and bereavement teams as needed
* Participate in quality improvement initiatives, training efforts, and team meetings
* Provide on-call support when assigned
* Assist with discharge planning and referrals
*Qualifications*
* Master's degree in Social Work from an accredited institution
* LCSW (or eligibility) required
* At least one year of social work experience; hospice or home care experience preferred
* Experience with grief counseling or bereavement work strongly preferred
* Valid New York State driver's license
* Strong communication, interpersonal, and crisis-intervention skills
* Compassionate, patient-centered approach and commitment to mission-driven care
