*NORTH LAKE CARE CENTER, *an 85 bed Skilled Nursing Facility, is seeking a *PART TIME PHYSICAL THERAPIST *to join our team!

*Pay: $60 per hour *
*Summary of the Physical Therapist (PT):*
The Physical Therapist will carry out an individualized program of physical therapy including assessment, treatment, planning, implementation, education, and communication to maximize a patient's progress toward achieving functional goals. The Physical Therapist will also maintain a positive working relationship with other health professionals and ancillary staff.
*Essential Duties & Responsibilities of the Physical Therapist (PT):*
* Creates, implements, and executes patient treatment plans.
* Communicates with patients, families, referring physicians, and other members of the team.
* Provides timely written documentation per facility and department requirements and as required by insurance provider to meet licensing, certification, regulatory, and reimbursement requirements.
* Develops and enhances clinical and professional skills through knowledge and professional associations.
* Supports and participates in department operations and development.
* Reports any changes in patient status or needs to Director of Rehab and interdisciplinary team as appropriate.
* Supervises Physical Therapy Assistants with the goal of ensuring optimal patient care and achievement of outcomes.
*Additional requirements of the Physical Therapist (PT): *
* Possessing a Physical Therapy degree and licensed in the state of Florida
* CPR Certified
* Physically able to assist individuals
* Moderate level of computer literacy
* Possess strong listening, speaking, critical thinking and reading comprehension skills
* Socially perceptive
* Strong time-management skills
