* Collaborate with other healthcare professionals to ensure cohesive patient care.
* Provide education to patients and their families regarding treatment plans and home exercises.
* Participate in discharge planning to facilitate smooth transitions for patients leaving care.
* Maintain accurate documentation of patient progress and treatment outcomes.
* Stay current with medical terminology and best practices in physical therapy.
*Requirements*
* Doctor of Physical Therapy (DPT) degree from an accredited program.
* Valid state licensure or eligibility for licensure as a Physical Therapist.
* Experience in outpatient settings preferred; familiarity with geriatrics, orthopedics, and home health is a plus.
* Strong knowledge of discharge planning procedures and care plans.
* Ability to work effectively within a multidisciplinary team environment.
* Excellent communication skills for interacting with patients, families, and healthcare staff.
* Proficient in medical terminology related to physical therapy practices.
* Commitment to ongoing professional development and staying updated on industry advancements.
