We are seeking a dedicated and skilled Occupational Therapist to join our hospital-based rehabilitation team. The OT will assess and treat patients with physical, cognitive, or psychosocial impairments resulting from illness, injury, or surgery. The primary goal is to improve patients' independence and safety with daily activities during their hospitalization and prepare them for discharge to home or other care settings.
*Key Responsibilities:*
* Conduct comprehensive OT evaluations for patients with a variety of diagnoses (e.g., stroke, orthopedic injuries, neurological disorders, post-surgical conditions).
* Develop and implement individualized treatment plans aimed at improving functional independence and ADLs (Activities of Daily Living).
* Recommend appropriate adaptive equipment and provide training for its use.
* Collaborate closely with multidisciplinary teams, including physicians, nurses, physical therapists, case managers, and speech-language pathologists.
* Document treatment plans, progress notes, and discharge summaries in compliance with hospital, legal, and professional standards.
* Participate in discharge planning by assessing patients' home environments and recommending appropriate support or further rehab services.
* Educate patients and caregivers about exercises, safety, and continuing therapy needs after discharge.
*Qualifications:*
* Bachelor's, Master's, or Doctorate degree in Occupational Therapy from an accredited program.
* Current state licensure
* Certification from NBCOT (National Board for Certification in Occupational Therapy).
* CPR/BLS certification (required).
* Strong organizational and communication skills.
